TELEHEALTH:
Using the Health Buddy® appliance in the home transmits important patient health information and self-regulating disease management programs between the patient's home and Caris RNs. By collecting individualized patient information daily, Telehealth Advisor provides Caris with continually updated electronic patient record-giving an understanding of the patient's health on a day-to-day basis. This information is then available to each member of the patient's care team, which improves care coordination and makes it possible to provide more proactive, targeted care.
